A sign up fee of $250 per family or per individual will be charged at the time of membership purchase. This fee will be charged once for families who sign up together where family is defined as parents and their dependent children under the age of 18. This fee will not be charged for membership renewals. The sign up fee is non-refundable. The family sign up fee is limited to a maximum of 6 individuals.
All facility memberships require a minimum 3 month sign up period. Those who choose to sign up with a monthly payment plan will be charged for those 3 months in advance and will be charged monthly thereafter.
Payments will be processed at the beginning of each month and will constitute payment for the entirety of that month and ending on the last day of the month.
There are no early cancellation fees and memberships may be cancelled at any time. In the event of a membership cancellation the purchaser will lose access to the clinic at the beginning of the month that follows the completion of the cancellation request.
No refunds will be made for any remaining time on cancelled memberships irrespective of the amount of time remaining on the membership.
No refunds will be granted if the clinic services can not or have not been used.
Advantage Medical holds the right to terminate memberships in situations where payment of membership fees are late, refused or missed. In the event of a missed payment, a reminder will be sent to the membership account holder. Failure to complete payment within 2 weeks of the initial due date will result in automatic termination of the membership.
Advantage Medical holds the right to terminate memberships at any time for violations of clinic policies. Advantage Medical may terminate memberships without notice and without explanation. Refunds will not be provided for memberships that are terminated.
Yearly and Monthly Advantage Membership plans may be upgraded at any time by the purchaser. A top up payment will be made immediately to make up the difference between plans and members may access services available in the new plan immediately. In the case of upgrading to a monthly Advantage Plus Memberships, Advantage health spending accounts will not be accessible until 3 months after plan upgrades are completed. If the purchaser upgrades to a yearly Advantage Plus Memberships the Advantage spending account will be made available once the membership payment has been processed.
Due to the services offered in the Advantage Plus Memberships plans, yearly Advantage Plus Memberships are not able to be downgraded. Monthly Advantage Plus Memberships may be downgraded after a period of 3 months.
Monthly Advantage Plus Memberships plans must be held for a minimum of 3 months prior to acquiring the Advantage Clinic Spending Account. Yearly Advantage Plus Memberships plans will be granted access to the Advantage Clinic Spending Account once the membership payment has been processed.
Memberships are not transferable and can only be used by the purchaser. Membership sharing is not allowed, and any infractions may result in immediate termination of the purchasers membership without refund.
Membership cards are provided free of charge. Replacement cards can be provided if cards are lost or broken. A charge for the value of the card will be made to the purchaser’s account. Cards are not to be shared and doing so may result in membership termination. Members who choose to cancel their membership will have their card access revoked and all Advantage cards owned by the member must be returned to Advantage Medical. We ask that any lost cards be reported immediately to Advantage Medical so that the card access can be revoked.
Fees for Advantage Programs are strictly for the uninsured components of care, including non-physician care. Physician visits and after-hours care, prescribed tests, and any other publicly insured services will not be billed to our patients. Subscription to our Advantage Program is not a precondition to access insured physician services from the physicians at our clinic.
